Man and dog died

I saw in todays paper about the tragic loss of life of a young man who tried to rescue his dog which had fallen over a cliff. It appears that both he and his girlfriend fell, but the man died, so did the dog, and the lady is severely injured in hospital. To me, this is tragic, but was also preventable.
I've heard so many times in the news, of dogs chasing seagulls, or balls, or just not looking where they were going even, and then falling off cliffs, it happens every year. But what I can't understand is, why do owners still take the risk? Would you let small children play on the edge of a cliff?
Why on earth don't they pop the leads on till they are somewhere safer? A 10 seconds job would prevent these sad events.

Accidents, by definition are unexpected, if common sense prevailed in such circumstances there would be no accidents to report.

I have several potential accidents everyday, and after each one I think, "I knew that would happen", but more importantly, it don't stop me doing stupid stuff.

Well I like to imagine that clipping a lead on isn't beyond anyone's control?

It is if the individual concerned does not perceive not clipping on a lead as a problem.
